MISSION STATEMENT

The mission of the Community Based Outpatient Clinic is to function as an extension of the Ambulatory Care Service of the Louis A. Johnson VA Medical Center, Clarksburg, WV, in order to provide selective outpatient and follow-up medical care to the eligible veterans in the clinic catchment's area.

PURPOSE

The purpose of the clinic is to provide selected medical services on an outpatient basis to eligible veterans in certain western counties of WV and eastern counties of Ohio. It is important to understand that the clinic is not equipped for emergency medical services.

WHO IS ELIGIBLE FOR CARE?

  • * Any veteran with a rated service-connected disability.
  • * Any Ex-POW for any condition.
  • * Any veteran of WWI or the Mexican Border period.
  • * Any veteran who receives aid and attendance or housebound benefits for any conditions.

  • * Any non-service-connected veteran who was released or discharged from military service under honorable conditions may be provided medical treatment or hospitalization based upon income (means test).

  • WHAT SERVICES ARE PROVIDED?

  • Diagnostic testing - Routine blood work and EKG.

  • Social Work services - one day per month.

  • Patient Education & Preventative Medicine

  • VA Contact Representative

  • HOW DO I REGISTER OR SCHEDULE APPOINTMENTS?

    If you have never been seen at the Community Based Clinic, call 1-304-478-2219 for instructions on how to register or schedule appointments.

    MEDICATION

    The VA has the authority to collect a $2 co-payment for each 30 day or less supply of medication when the prescription is for a non-service-connected condition. Only a limited supply of medications is provided at the clinic with most being mailed to the patient from the VA Medical Center at Clarksburg. Medications are not provided for prescriptions written by physicians outside the VA facility. We only fill prescriptions when you, as a patient, are under the care of a local VA physician, and the prescription has been authorized by that physician.
