Safety Committee Minutes April 12, 2006 President’s Conference Room In attendance: Christine Post, Lennie Moore, Gary Waggoner, Dave White, Cathy Mutz, Sandy Swisher – guest, Jan Miller, Nelson Ortiz, Joe Badgley, John Tharp, Paul Milhoan, Craig Rabatin – Chair, Mary Lott, and Marie Foster-Gnage. Absent: Dan Williamson, Al Collins, Charlene McNulty, Jack Simpkin Craig Rabatin, Chair, called the meeting to order. The minutes were accepted as written. Sandy Swisher had attended as a guest to discuss the revised Accident/Injury Form, the agenda was set aside to discuss. WVU ID. Sandy Swisher indicated that the WVU ID could be obtained from WVU EHS web site. Many people are entering Social Security Numbers which should never be used. The WVU ID number replaces the Social Security Number. These numbers are given to employees and students at WVU. Students at WVU Parkersburg have not been given a number. The Assignment Number is an Oracle assignment number given to employees by WVU. It was agreed by Sandy Swisher and the committee that this portion of the form be left blank and could be entered by the HR office when they receive the form. This will be noted in the Instruction sheet for completion of these forms. Training: Dave White noted that Larry Stephens will be getting with the Safety Committee to discuss "What to say and what not to say" after an accident. This will assist in limiting liability issues. Nelson Ortiz reported to the committee on completion of the form from the Campus Police/Security stand point. He reported that Campus Police should be called to the scene of any accident. The scene is crime scene until properly investigated, and all accidents are to be reported through Central. Case numbers will be assigned and included on the Accident Forms. It is noted that Campus Policy/Security are to forward a copy of their Incident Report to the Safety Officer. The Safety Officer then forwards a copy to HR for employees and to the Records Office for students. All records are to be maintained by the Safety Office. Safety Committee Minutes April 12, 2006 Page Two Only Administrators or supervisors are to complete the reports after fact finding from victims, witnesses and campus police reports. The injured party signs off on the completed form but does not complete the form. Nelson reported to the committee that he will be working with Dave White on a training session regarding accident and the reporting process. The President suggested that we limit the number of forms and keep the process as simple as possible. The first responder at the scene calls for help then calls for assistance from college personnel. If an injured person refuses to be transported via ambulance, this is to be noted on the Accident Report. Any person who transports an injured person has to understand that they are accepting responsibility and assuming the liability. The ambulance service should be noted on the Accident Form. The President Gnage says if it’s in writing, then only the facts should be written. This training will be required for all administrators. A possible date for the training was discussed with one date being May 10. OLD BUSINESS: Emergency Response Procedures Draft The draft was prepared by Debbie Richards and Cathy Mutz some time back, but never finished from lack of input. It is important to get this finalized. A committee consisting of Cathy Mutz, Debbie Richards, John Gorrell, Craig Rabatin, Dave White and Mary Lott will be putting together a final draft form. This should be done as quickly as possible. Goal date for this completion of draft is May 13. Goal date for approval of final Emergency Response Procedures is September, 2006. HIPPA should be included in the draft. Project Sheet: COMPLETE. Emergency phone numbers were handed out to the Committee and have been given to Debbie Richards for inclusion in the Answer Book policy. A copy will be forwarded to Connie Dziagwa for inclusion on the college web page. Safety Plan—Mary will make some changes in the plan and forward plan to the committee ASAP. 29. COMPLETE. Equipment and installation of broadcast/evacuation system is quoted to be $131,000. Safety Committee has recommended that a grant be pursued from Homeland Security to obtain this much needed equipment. Phone service at switchboard—Dan Williamson was out of town and could not report on this project. 32. Database for training—Brandi Marlow was out of town and could not attend to report on this project. Mary Lott did give the committee limited information on how easy it will be to enter employee information on training into the data base. 34. Web Site development—a continuing project to be discussed with the President. TRAINING: CHO Training. John Tharp noted that the WVU has indicated that they would like to come to the campus for the training. Haz-Mat training will be offered annually in the fall—How to read pamphlets may be put on web site. First Aid—Charlene McNulty is planning for employees. Some first aid kits have been distributed. Chemical Storage & Handing—Training session is in the planning CPR/AED—Charlene McNulty is planning. Forklift training—Done annually by WVU at our campus Fire Extinguisher—Fall 2006 Blood Borne Pathogens—no report this meeting Wellness for staff—Videos available—Dave White to let us know when these are available for staff. What to do after an Accident?—In the planning stages. Fire Training—Kenny Kline has visited the campus. Employee training was suggested. Electrical Licensing—Done annually Asbestos—taking samples, done annually Hilti Training—licensing for maintenance staff Fit Testing—annually for maintenance staff CHO discussion: President indicated that there is only one CHO and that is John Tharp. It is noted that his services are used occasionally by other areas, but these are very limited requests. Based on the size of the institution one CHO would be enough for the occasional spill when there are others who are knowledgeable in this same area. Training should be such that it would cover all the chemicals in a specific area—MSDS sheets. Safety records should be with Safety Officer. MSDS should be in all areas—there should be a Master Book.
Safety Committee Minutes April 12, 2006 Page Four Lab plans are unique to each area—journalism, print shop, science labs (biology, chemistry). Need to take out information that is old and replace with new. Lab plans should read: Chemical Hygiene Plan for (insert area). Emergency phone number sheets are white with red letters. The information is part of your meeting material.
SAFEY PLAN In November we associated names with specific sections of the Safety Plan. There are still some of those sections which have not been complete. This needs to be completed as soon as possible. We need to set a date for completion. ADDITIONAL DISCUSSION ITEM: A discussion of creating sub-committees to do inspections of specific areas was discussed. These committees would have assigned areas to inspect and report back to the Safety Committee any problems or situations that would require additional attention.
